Certified & End of Life Experienced Staff
A Guided Living Registered Nurse with hospice experience will assess the client and collaborate with hospice agency colleagues to assure the most optimal end-of-life experience as possible in the environment of choice (home, assisted living, nursing home).
Guided Living caregiving staff (Certified Nursing Assistants and Certified Home Health Aides) are specifically trained and experienced in provision of holistic centered care for our end-of-life clients.
Care Plans Mindful of Dignity & Comfort
Each individual care plan is created by a Registered Nurse with end-of-life hospice experience with input from loved ones. Every ‘task’ of the care plan considers the person’s dignity and comfort while honoring their expressed choices for care at end-of-life.
Therapeutic Holistic Presence
Utilization of therapeutic holistic presence is the primary foundation of this specialized care that provides comfort, safety and ease of transition. Holistic Presence is provided by adhering to Watson’s Caritas Model of Care By:
